Job Summary
Join our dedicated healthcare team as a Home Health Registered Nurse, where you will play a vital role in delivering compassionate, personalized nursing care directly to patients in their homes. As a Home Health RN, you will coordinate care plans, educate patients and families, and serve as a trusted advocate within diverse communities. Responsibilities
- Assess patients’ health conditions through comprehensive evaluations and develop individualized care plans aligned with physician directives.
- Administer medications, treatments, and wound care while monitoring patient responses to ensure safety and effectiveness.
- Provide education on disease management, medication adherence, and health maintenance strategies to patients and their families.
- Coordinate with social services, community resources, and healthcare providers to facilitate access to additional support such as Medicaid, Medicare, or Section 8 housing programs.
- Conduct crisis intervention and crisis management when urgent situations arise, ensuring prompt response and appropriate escalation.
- Document all patient interactions accurately in electronic health records while maintaining confidentiality and compliance with healthcare regulations.
- Engage in ongoing professional development including workforce development initiatives, mentoring new staff, and staying current on best practices in behavioral health and social work.
Skills
- Strong knowledge of crisis intervention techniques and addiction counseling strategies.
- Experience working with geriatric patients, seniors, individuals with disabilities, and culturally diverse communities.
- Proficiency in social services coordination including resource identification within social service agencies.
- Certified in CPR (Cardiopulmonary Resuscitation) and first aid; ability to handle emergency situations confidently.
- Excellent client communication skills with the ability to motivate change through motivational interviewing techniques.
- Bilingual abilities are highly desirable for serving diverse populations effectively.
- Supervising experience or mentoring skills preferred for supporting team development.
- Familiarity with Medicaid, Medicare, Child Welfare programs, Childcare services, Section 8 housing policies, and workforce development initiatives.
