Position Summary
CWHA is seeking a dedicated and resourceful Linkage and Navigator Personnel to connect clients with the health and social support services they need to thrive. This role plays a critical part in supporting Brooklyn’s communities of color by improving access to care and removing systemic barriers.
The ideal candidate will bring experience in community-based healthcare navigation, a strong commitment to health equity, and exceptional interpersonal skills. The role involves direct client interaction, service coordination, advocacy, and collaboration with local healthcare and social service providers.
Key Responsibilities
Participant Navigation & Support
Serve as the primary liaison for clients, offering guidance through available health and social service systems.
Conduct comprehensive participant assessments to identify needs and develop tailored referral plans.
Provide consistent follow-up and support to ensure clients access and remain engaged with recommended services,
Service Linkage & Referral Coordination
Maintain a current directory of culturally sensitive healthcare providers, social service agencies, and community partners.
Coordinate timely and appropriate client referrals, ensuring a seamless connection to services and accurate documentation of all interactions.
Community Engagement
Represent CWHA at outreach events, community workshops, and health fairs to raise awareness of available services.
Build and sustain partnerships with local stakeholders to enhance referral pathways and support holistic care delivery.
Recordkeeping & Reporting
Maintain accurate and up-to-date records of client engagements, referrals, and outcomes in designated program databases.
Collect and report service utilization and client satisfaction data, contributing to program evaluation and improvement efforts.
Collaborate with program staff to continuously improve the quality and impact of navigation services based on client feedback.
Qualifications
High school diploma or equivalent required; Associate’s or Bachelor’s degree in social work, public health, human services, or a related field preferred.
Minimum of two (2) years of experience in healthcare navigation, case management, or community outreach, preferably within underserved populations.
Strong understanding of sexual and reproductive health services, with sensitivity to the cultural needs of communities of color.
Excellent communication and interpersonal skills with the ability to build rapport and trust with diverse individuals.
Familiarity with Brooklyn’s healthcare and social service infrastructure.
Proficient in maintaining client records and basic data reporting.
Bilingual applicants (e.g., English/Spanish, English/Haitian Creole) are highly encouraged to apply.
Must be able to travel locally within Brooklyn to support clients and attend community events.
