Duties
- Assist patients with activities of daily living including bathing, dressing, grooming, feeding, and mobility to promote independence and dignity.
- Monitor vital signs such as blood pressure, pulse, temperature, and respiration rates accurately and report changes promptly.
- Support clinical procedures by preparing patients and assisting with tasks like catheter care, wound dressing changes, and basic medical documentation.
- Maintain a safe environment by following aseptic techniques, infection control protocols, and proper use of equipment like Hoyer lifts and other mobility aids.
- Document patient information meticulously using Electronic Health Record (EHR) systems such as Epic or Cerner to ensure accurate medical records.
- Collaborate with interdisciplinary teams including nurses, physicians, therapists, and social workers to coordinate comprehensive patient care plans.
- Provide emotional support and companionship to individuals with dementia or behavioral health needs while respecting their dignity and preferences.
Experience
Candidates should demonstrate prior experience working in nursing homes, assisted living facilities, or home health agencies. Experience in specialized areas like Alzheimer’s care, memory care, hospice support, pediatric or behavioral health is highly valued. Familiarity with medical terminology, medication administration protocols—including phlebotomy—and proficiency with EMR/EHR systems like eClinicalWorks or Cerner is preferred. Candidates with critical care experience such as ICU or Level II trauma center exposure will stand out. A strong understanding of physiology, anatomy knowledge, HIPAA compliance for patient privacy, and competence in medical documentation are essential for success in this role.
