American Traveler is seeking a licensed Physical Therapist with a minimum of 1 year of home health experience for a 13-week home health assignment in the Lincoln, NC area.

Home health setting serving patients across multiple communities including Lincolnton, Belmont, Denver, Mount Holly, Cramerton, and surrounding areas
-
Typical caseload is 6 patient visits per day, with an average driving distance of 60–80 miles per day
-
Patient population includes chronic disease management and orthopedic/post-surgical cases
-
Day shift schedule, Monday–Friday, 8:00 AM – 5:00 PM, 40 hours per week
-
No weekend rotation, no on-call, and no major holiday coverage required

Uses HCHB (Homecare Homebase) as primary documentation platform; Epic Rover available for view-only access; Focura used for uploading discharge instructions and wound photos

A company cell phone (Samsung Note 9) is provided for documentation and patient/provider communication

Mileage is reimbursed weekly; no personal company vehicle provided

Minimum 1 year of home health PT experience required, with most recent experience within the last year

Active NC State PT license required; license required if applicable

Current BLS certification required

Valid Driver's License and proof of auto insurance required

Experience with HCHB (Homecare Homebase) required

Last 4 digits of SSN and full date of birth required for consideration

Minimum 1 professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN

Candidates must reside at least 50 miles from the home unit to be eligible; local candidates are not accepted

Responsibilities include patient evaluation, reassessment, discharge planning, and supervision of PTAs

Therapists schedule patient visits the day prior to the planned visit
-
Required visit equivalent (VE) minimum is 30 per week; cancelled appointments do not count toward VE totals
-
5-day facility orientation provided, including new employee orientation (NEO)
-
Supported by departmental leadership including a Clinical Manager, Clinical Supervisor, Scheduler, and a collaborative clinical team

Grey scrubs required

A manager interview is required as part of the selection process

First-time travelers will be considered if they have seasoned home health experience

Candidates who have worked a permanent position at an affiliated facility must have been separated for at least 1 year before being considered
