Job Title: Family Case Manager
Position Summary:
This a full time, contractual, position with Anne Arundel County Partnership for Children Youth and families. The position provides intensive case management services to families at risk of homelessness and those experiencing homelessness in Anne Arundel County. The person in this position reports to the Family Stability Deputy Director.
Responsibilities:
A. Secure housing for families using a rapid rehousing model that includes:
- Flex funding for first and last month's deposit and sliding scale rent payment,
- Financial literacy training including monthly money management and bill paying,
- Avenues to address under-employment/seasonal employment,
- Removal of barriers that prevent permanent housing.
B. Work with families at risk of homelessness offering resources and support including:
- Identify strengths and needs using the Arizona self-assessment
- Clarify and prioritize the issue(s),
- Provide emotional support,
- Identify available resources, both formal supports and informal support
C. Facilitate family access to resources (public and private sector):
- Coach and support families
- Support basic needs including housing, good and other emergency services
- Provide guidance in completing forms and applications
- Review documents to ensure family understands; documents include but are not limited to:
- *Individualized Education Programs (IEPs)/504 Plans
- *Mental Health Assessments
- *Court Orders
- Empower families to articulate their concerns and needs effectively.
Qualifications:
A. Has a Bachelor’s degree in a behavioral sciences or related field, or
B. A combination of at least three years work, education, training, or experience; and
C. The following skills and abilities:
Knowledge, skills and abilities
- Effective communication, written and oral.
- Knowledge of Anne Arundel County
- Experience with available resources and services in Anne Arundel County
- Ability to work independently.
- Ability to work with families/youth of diverse cultural, religious, racial, educational, and socio-economic backgrounds.
- English-Spanish oral and written bilingual proficiency preferred
- Proficiency with Google software and Apricot or related case management software systems preferred
- Ability to skillfully engage with clients while also maintaining professional boundaries
- Ability to adapt and assimilate new information as resources, laws, and policies evolve.
Special Conditions or Requirements:
- Must have reliable transportation: required to travel to attend meetings, activities and programs; current driver’s license and insurance.
- Must have an approved criminal background check.
- Must be able and willing to schedule work in a flexible manner, including some evenings and weekends.
Job Types: Full-time, Contract
Pay: $60,000.00 - $75,000.00 per year
