Schedule: Monday–Friday, 8:00 AM – 5:00 PM
Job Description
We are seeking a compassionate and experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout the Milwaukee, WI area. The RN will assess patients, develop and implement individualised care plans, coordinate with interdisciplinary teams, educate patients and caregivers, and ensure high-quality, patient-centred care in accordance with physician orders and agency policies.
Key Responsibilities
- Provide skilled nursing care in patients' homes following physician orders and individualised care plans.
- Perform comprehensive patient assessments, including OASIS documentation.
- Monitor patient progress and revise care plans as needed.
- Educate patients and families on disease management, medications, self-care, and discharge planning.
- Coordinate care with physicians, RN Case Managers, therapists, and other healthcare professionals.
- Document clinical visits, assessments, and patient progress accurately and on time.
- Supervise LPNs, LVNs, home health aides, and other support staff.
- Participate in interdisciplinary team meetings and quality improvement initiatives (QAPI).
- Respond to changes in patient conditions and communicate updates to the healthcare team.
- Provide symptom management, rehabilitative nursing care, and end-of-life/palliative care when appropriate.
- Participate in rotating on-call coverage as assigned.
- Maintain compliance with federal, state, and agency regulations.
Required Qualifications
- Active Registered Nurse (RN) license in the State of Wisconsin.
- Minimum 2 years of RN experience providing skilled nursing care.
- Valid driver's license and current automobile insurance.
- Experience in bedside nursing, patient care management, symptom management, crisis intervention, and family education.
- Ability to work independently and collaboratively within an interdisciplinary team.
- Strong clinical assessment, communication, and documentation skills.
- Proficiency with tablet-based documentation systems.
Preferred Qualifications
- Previous Medicare Home Health experience.
- Experience with Homecare Homebase (HCHB) documentation system.
- OASIS assessment experience.
