Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and record patient vital signs such as blood pressure, pulse, and temperature using EMR (Electronic Medical Record) systems
- Support patients with medication administration following prescribed protocols while maintaining HIPAA compliance
- Perform patient observation to identify changes in condition or behavior, especially in individuals with dementia or Alzheimer's disease
- Provide specialized care for clients with developmental disabilities, hospice needs, or memory care requirements
- Operate medical equipment such as Hoyer lifts and catheters safely and effectively
- Assist with light housekeeping tasks including cleaning, laundry, meal preparation, and maintaining a safe environment
- Document medical records accurately and ensure all documentation complies with healthcare regulations and standards
Requirements
- Proven experience in home health care, nursing homes, assisted living or long-term care environments
- Knowledge of medical terminology and procedures related to patient care and safety
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care or dementia training are a plus
- Ability to work independently while following detailed care plans and medical instructions
- Experience working with individuals with disabilities or special needs preferred
- Familiarity with EMR/EHR (Electronic Health/Medical Records) systems for documentation purposes
- Strong communication skills to collaborate effectively with patients, families, and healthcare teams
- Valid driver’s license and reliable transportation are often necessary for home visits
