Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems such as Epic or Cerner.
- Administer medications and injections accurately, including IV infusions and catheter care, following strict protocols to ensure patient safety.
- Assist with complex procedures such as tube feeding, wound suturing, and sterile processing while maintaining aseptic techniques.
- Support acute and chronic care management by monitoring patients with conditions like Alzheimer's disease, dementia, or infectious diseases in settings such as long-term care facilities or hospitals.
- Collaborate with interdisciplinary teams on discharge planning, case management, and triage to optimize patient outcomes across outpatient clinics or inpatient units.
- Conduct diagnostic evaluations including specimen collection/processing and medical imaging support as needed.
- Maintain meticulous medical records and documentation in compliance with HIPAA regulations and long-term care standards.
Skills
- Proven experience working with electronic health records (EHR) systems such as Epic, Athenahealth, or eClinicalWorks.
- Strong background in critical care environments including ICU, emergency room, or Level I trauma center experience.
- Skilled in advanced nursing procedures like IV insertion, phlebotomy, catheterization, and ventilator management.
- Knowledge of ICD-9/10 coding for accurate medical documentation and billing processes.
- Expertise in caring for geriatric populations with dementia or memory care needs within skilled nursing facilities or assisted living communities.
- Familiarity with infection control practices and acute pain management strategies.
- Excellent client communication skills to effectively educate patients and families about treatment plans and health coaching.
