Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and document patients’ vital signs and health status regularly to ensure optimal care
- Administer medications accurately following care plans and HIPAA privacy regulations
- Prepare nutritious meals tailored to dietary needs and assist with meal consumption as needed
- Support patients with memory care, dementia, Alzheimer’s, or other cognitive challenges through patient observation and engagement
- Operate equipment such as Hoyer lifts or other assistive devices to ensure safe transfers and mobility
- Maintain a clean and safe environment by performing light housekeeping tasks like cleaning and laundry
- Follow prescribed care plans developed by healthcare professionals to deliver consistent, high-quality care
- Communicate effectively with family members, healthcare teams, and social workers to coordinate ongoing support
- Provide emotional support and companionship to foster a positive environment for clients
Skills
- Proven experience in home health, assisted living, nursing homes, or group home settings with a focus on senior or disability care
- Strong knowledge of patient observation, monitoring vital signs, medication administration, and dementia or Alzheimer’s care
- Certified in CPR and First Aid; familiarity with HIPAA regulations is essential for maintaining privacy standards
- Ability to perform heavy lifting safely using equipment such as Hoyer lifts when necessary
- Experience working with individuals with developmental disabilities or autism spectrum disorders is highly valued
- Excellent communication skills for collaborating with healthcare teams and supporting patients’ emotional needs
- Demonstrated ability to follow detailed care plans while adapting to individual client needs
- Physical stamina for long shifts involving mobility assistance and household chores
