Registered Nurse (RN) – Home Health | 13-Week Travel Contract
Location: Phoenix, AZ
Start Date: July 27, 2026
Contract Length: 13 Weeks
Schedule: Monday–Friday | 8:00 AM – 5:00 PM (5x8 Days)

Key Responsibilities:
- Perform Start of Care (SOC), follow-up, recertification, and discharge visits.
- Complete patient assessments and develop individualized care plans.
- Provide wound care, medication management, patient education, and skilled nursing services.
- Coordinate care with physicians, therapists, caregivers, and other healthcare professionals.
- Supervise and evaluate Home Health Aides (HHAs) and LPN/LVN staff as assigned.
- Complete all documentation accurately within required timeframes.
Requirements::
- Active RN license (license must be in hand at submission).
- Minimum 1 year of recent Adult Home Health experience or Hospice experience.
- Current BLS/CPR certification.
- Proficiency with HomeCare HomeBase (HCHB) within the last 6 months.
- RN candidates must be proficient in OASIS documentation.
- Valid driver's license, reliable vehicle, and current auto insurance.
- Ability to travel up to 75 miles from the assigned care center.
- Recent home health experience required; first-time home health travelers will not be considered.
Assignment Details:
- Average Caseload: 5–8 patients per day.
- Productivity Expectation: 30 points per week.
- Types of Visits: Start of Care, Wound Care, Follow-Up Visits, Recertifications, and Discharges.
- Mileage Reimbursement: IRS standard rate between patient visits.
- No weekend rotation required.
- No guaranteed hours.
- Local candidates accepted at the same rate.
EMR: HomeCare HomeBase (HCHB)
Important Notes
- Home Health Checklist required.
- Documentation must be completed within 24 hours of each patient visit.
- Clinicians must accept assigned patients regardless of travel distance within the territory.
- Travelers may be floated to nearby clinics if patient census is low.
