Maintains confidentiality of all agency related information
· Monitors and maintains compliance with all applicable licensing and regulatory standards
· Requests and receives all relevant child records (social histories, psychological, educational records, medical records, etc.).
· Conducts personal interviews with the referring worker in order to compile a social history
· Assists in matching children to best suited foster families
· Participates in pre-placement interviews with the child, foster family, and referring worker
· Facilitates team meetings and case conferences (consisting of the referring agency, biological parents, therapist, child, and foster parents)
· Completes all documentation required for a client including but not limited to; Intake Studies, Individual Service Plans, and Progress Reports, Narratives, Treatment plans.
· Completes and maintains documentation for foster families of clients assigned.
Completes home studies for foster families
· Arranges for psychological, educational, dental, and medical testing as needed
· Visits with the child and foster family at a minimum of twice per month
· Maintains ongoing contact with the child and foster family throughout placement
· Provides foster families with support and recommendations regarding behavior management and any other needed community resources
· Maintains contact with the biological family to prepare them for the placement permanency plan goal (when necessary)
· Assists with school enrollment and any other educational services
· Completes and maintains all documentation within the child’s record
· Attends mandatory staff meetings
· Attends professional conferences and seminars
· Covers "on call" schedule when necessary
· Performs all other duties as assigned by the Director of Social Services
· Ongoing training related to the job position
Job Type: Part-time
Pay: $18.00 - $28.00 per hour
