Job Summary
Suburban Healthcare & Rehabilitation is seeking a PRN Physical Therapist (PT) to provide therapy services on an as-needed basis. This role supports resident care by evaluating, treating, and helping residents improve mobility, strength, and functional independence in a skilled nursing setting.
Responsibilities
- Perform physical therapy evaluations and treatments per physician orders
- Develop and carry out individualized treatment plans
- Document evaluations, treatments, and progress notes accurately
- Communicate with nursing staff and the interdisciplinary team
- Educate residents and families on mobility, exercises, and safety
- Follow all facility policies and state and federal regulations
Qualifications
- Current Ohio Physical Therapist license
- Graduate of an accredited Physical Therapy program
- Skilled nursing or long-term care experience preferred
- Strong documentation and communication skills
- Reliable, flexible, and able to work independently
Position Details

- No guaranteed hours
- Coverage for weekends, vacations, or call-offs as needed
