Shift: Monday–Friday, 8-hour day shifts, until work is complete (40 hours per week)
Pay: $2300 - $2440 weekly gross for traveler ($51/hr - $53/hr for local)
Job Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) specializing in Home Health to join our healthcare team. Duties
- Conduct thorough patient assessments, including vital signs, medical history review, and physiological evaluations, to determine individualized care plans.
- Deliver direct nursing care such as medication administration, wound management, IV therapy, and monitoring of acute and chronic conditions.
- Coordinate discharge planning and transition of care from hospital or clinic to home settings, ensuring seamless continuity of care.
- Manage patient records accurately using Electronic Health Records (EHR) systems while adhering to HIPAA regulations to protect patient privacy.
- Collaborate with healthcare professionals—including physicians, therapists, and social workers—to develop and implement effective treatment strategies.
- Educate patients and their families on disease management, medication adherence, and health promotion techniques.
- Perform diagnostic evaluations and assist with medical procedures as needed within the scope of home health nursing.
- Participate in utilization management and case management activities to optimize resource use and improve patient outcomes.
- Monitor patient progress regularly through remote or in-person visits; adjust care plans based on clinical findings.
- Facilitate communication between patients and healthcare providers to ensure clarity regarding treatment goals and expectations.
Qualifications
- Valid Registered Nurse (RN) license with current state certification.
- Proven experience in critical care environments such as ICU, emergency room, trauma centers (Level I & II), or hospital medicine.
- Demonstrated expertise in post-acute care settings including hospice, rehabilitation, or home health nursing.
- Strong knowledge of medical terminology, physiology, infectious disease management, and health regulation policies.
- Experience with EMR/EHR systems for documentation and medical records management.
- Familiarity with managing complex cases involving behavioral health, geriatrics, pediatrics (PICU), or outpatient clinics.
- Excellent patient service skills with the ability to communicate effectively across diverse populations.
- Ability to perform comprehensive patient assessments and clinical case management efficiently.
- Knowledge of HIPAA compliance standards and medical records confidentiality protocols.
- Additional experience in managed care environments or outpatient clinics is advantageous.
