Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor and record vital signs such as blood pressure, pulse, and temperature to track health status
- Administer medications accurately following care plans and HIPAA regulations to ensure safety and compliance
- Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation
- Prepare nutritious meals tailored to dietary needs and assist with light housekeeping tasks like cleaning and laundry
- Support clients during outings or appointments within the community, ensuring safety at all times
- Use specialized equipment such as Hoyer lifts or resident lifts to assist with mobility and prevent injury during transfers
Skills
- Proven experience in home health, assisted living, or long-term care environments with a focus on senior or disability care
- Ability to perform CPR and First Aid confidently in emergency situations
- Familiarity with working under HIPAA guidelines to maintain client confidentiality
- Strong skills in medication administration, vital signs monitoring, and developing or following care plans
- Experience working with individuals with developmental disabilities, autism, or other special needs populations
- Physical stamina for heavy lifting and assisting with mobility devices such as Hoyer lifts or wheelchairs
- Excellent communication skills for effective social work interactions and collaboration with healthcare teams
