Summary
The Licensed Practical Nurse (LPN) plays a vital, dual-role position combining clinical oversight with operational support for our non-skilled home care services. In this role, you will serve as the primary clinical liaison between clients, caregivers, and administrative staff to ensure care plans are executed safely, effectively, and in strict compliance with state regulations.
Additionally, this position acts as a critical operational safety net—providing urgent, direct care support in the field when unforeseen staffing shortages arise to guarantee continuity of care for our clients.
Essential Job Duties AND Responsibilities
Clinical Oversight and Care Coordination:

Conduct initial client assessments and develop personalized, non-skilled care plans in collaboration with the client, family, and other healthcare professionals.

Perform regular quality assurance visits and phone calls to monitor the effectiveness of care plans and ensure client satisfaction.

Act as the primary point of contact for clinical questions and concerns from clients, caregivers, and families.

Provide guidance and training to non-skilled caregivers on proper care techniques, client-specific needs, and documentation protocols.

Ensure all client documentation, including care plans and visit notes, is accurate, complete, and maintained in accordance with agency standards.

Address and resolve clinical issues, escalating complex cases to a Registered Nurse (RN) or management as needed.

Create and manage reports within HHA Exchange
Operational Support and Direct Care Coverage:

Serve as a professional fill-in caregiver on an as-needed basis to ensure seamless service provision during staffing shortages, call-offs, or other scheduling challenges.

Provide hands-on care, including assistance with activities of daily living (ADLs), companionship, and light housekeeping, in alignment with established care plans.

Demonstrate flexibility and responsiveness in addressing urgent scheduling needs to maintain business continuity and client satisfaction.

Coordinate with Clinical Care Director
Professional Requirements:

Foundational understanding of medical conditions and terminology.

Ability to maintain a high level of professional conduct, including adherence to the agency's dress code; scrubs are permitted during home visits only.
