Job Summary
Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their homes. Responsibilities
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support.
- Monitor and record vital signs such as blood pressure, pulse, temperature, and respiration to track health status accurately.
- Support medication administration following prescribed schedules and medical documentation protocols while adhering to HIPAA regulations.
- Provide specialized care for individuals with dementia or Alzheimer's disease, including memory care strategies and behavioral support.
- Operate Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to document patient information securely and efficiently.
- Assist with catheter care, wound care, hospice care needs, and other medical procedures under supervision.
- Support patients with mobility using equipment like Hoyer lifts or other assistive devices; assist with transfers safely.
- Prepare nutritious meals and assist with meal planning that meets dietary restrictions or medical needs.
- Perform light housekeeping tasks such as cleaning, laundry, and maintaining a safe environment to promote overall health.
- Collaborate with healthcare teams to ensure comprehensive patient care plans are followed accurately.
Qualifications
- Proven experience working as a Home Health Aide or in long-term care settings such as nursing homes or assisted living facilities.
- Strong knowledge of medical terminology, patient observation techniques, and caregiving best practices.
- Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications like hospice care or dementia training are a plus.
- Experience working with individuals with developmental disabilities or disabilities is highly valued.
- Ability to operate EMR/EHR systems efficiently for documentation purposes.
- Knowledge of HIPAA regulations to ensure patient confidentiality at all times.
- Physical ability to lift patients using Hoyer lifts or similar equipment; capable of standing, walking, and performing caregiving tasks for extended periods.
