As a Licensed Practical Nurse (LPN) at our 50+ bed facility, you will play a vital role in delivering high-quality care to our residents. Under the direction of a Registered Nurse (RN), you will monitor, plan, and implement individualized nursing care plans to address each resident's unique needs. Your expertise will ensure continuity of care between shifts, and you will participate in resident and family education, advocating for their well-being and promoting our facility's mission.
Key Responsibilities:
- Monitor, plan, and implement individualized nursing care plans under the direction of a RN
- Ensure continuity of care between shifts by providing direct care and supervising C.N.A.s and support staff members
- Participate in resident and family education
- Serve as a resident advocate with a focus on our facility's mission
Requirements:
- Current and valid Wisconsin LPN license/registration
