Location: NY 13165
Pay: $1,258 – $1,420/week
Schedule: Part-Time Evenings, 3 shifts per week (rotational: 2 shifts wk1, 3 shifts wk2)
Additional: Every other weekend & holiday coverage required
Provide direct and indirect skilled nursing care to adult and geriatric residents
Collect and communicate resident data to RN and interdisciplinary team
Assist in development, evaluation, and revision of resident care plans
Collaborate with residents, families, and healthcare team to ensure safe, age-appropriate care
Maintain nursing knowledge and competency; mentor/orient staff as needed
Active LPN license (NY) – Required
BLS (AHA or RQI) – Required
Minimum 1 year LPN experience in a long-term care (LTC) setting
Knowledge of adult/geriatric care principles
