Duties
- Conduct thorough patient assessments to evaluate physical, emotional, and spiritual needs, utilizing your knowledge of physiology and medical terminology.
- Develop and implement individualized care plans aligned with hospice protocols and patient preferences.
- Completes an initial, comprehensive and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness( es).
- Administer medications and treatments as prescribed, monitoring for effectiveness and side effects while ensuring safety in compliance with HIPAA regulations.
- Ensures that arrangements for equipment and other necessary items and services are available.
- Provide education to patients and families about disease progression, symptom management, and comfort measures to support informed decision-making.
- Document all patient interactions accurately in medical records, including vital signs, symptom reports, and care updates.
Requirements
- Valid RN license with current registration in the state of practice.
- Valid Drivers License and active Car Insurance
- Current CPR certification
- On-call coverage rotation
- Critical care experience or ICU background preferred; experience in hospice or post-acute care is a plus.
- Demonstrated expertise in patient assessment, discharge planning, and medical records documentation.
- Knowledge of HIPAA regulations, medical terminology, and physiology essential for delivering high-quality care.
- Strong communication skills for effective patient service and collaboration within multidisciplinary teams.
- Ability to handle acute situations confidently while maintaining compassionate bedside manner.

Pay: $82,000.00 - $90,000.00 per year
