The Medication History Technician ensures safe and thorough patient care by collecting and documenting comprehensive medication histories for patients being admitted to the hospital.

Obtain complete and current medication histories from patients through direct interviews, caregiver conversations, and pharmacy or hospital record reviews
-
Conduct telephone interviews for upcoming surgery patients to gather medication information
-
Communicate with external pharmacies, physicians, and insurance companies to gather drug names, dosages, and administration details

Enter complete medication history into the hospital's electronic health record for physician review and reconciliation

Coordinate with nurses, physicians, and case managers to prioritize patients for medication interviews
-
Identify and resolve discrepancies in medication histories, escalating as needed to pharmacists or physicians
-
Perform quality audits of medication histories to ensure accuracy and completeness
-
Assist in the Emergency Department with medication distribution and monthly unit inspections
-
Maintain productivity standards in a dynamic and patient-centered environment
