Qualifications:
- Possess a Bachelor’s degree from an accredited college or university with a major in Social Work
- Must be eligible for or currently hold an LLBSW or LBSW license
- Possess a valid Michigan driver's license
Duties and Responsibilities:
- Comprehensive in-person assessment of physical and social/emotional functioning, medical status, physical environment and informal support potential.
- In cooperation with the participant or their designated representative, determine the selection and provision of services, establish their frequency and duration, and secure approval for each service from the participant or their representative as well as from physicians in accordance with specified requirements. Ensure the care plan receives final approval from the participant prior to implementation.
- Meet face to face with the candidate and allies to complete a NFLOC with Freedom of Choice to maintain participant’s eligibility. Enter the completed NFLOC into the State of Michigan electronic data system on the start of service date.
- Arrange and purchase health and social services in cooperation with the participant or his/her representative in accordance with the care plan.
- Serves as agent for negotiation, caregiving and monitoring formal service plans on behalf of participant.
- Provide contact with the participant and service provider as outlined in the care plan per contract requirements.
- A standardized in-person reexamination of the client's physical, social/emotional, medical status and informal supports is conducted every 90 to 180 days and re-enrollment as designated per Easterseals MORC policy.
