Job Summary

Duties
- Assist clients with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and toileting to promote comfort and dignity
- Monitor and observe patients’ conditions, noting changes in health status and reporting findings accurately using Electronic Health Record (EHR) systems
- Support medication administration following prescribed protocols while maintaining strict adherence to home healthcare regulations and HIPAA privacy standards
- Perform vital signs assessments including blood pressure, pulse, temperature, and respiration rates to track health trends
- Provide specialized care for clients with Alzheimer’s disease, dementia, or other memory impairments, including memory care techniques and behavioral management strategies
- Assist with mobility using tools such as Hoyer lifts or other assistive devices to ensure safe transfers and prevent falls
- Support hospice and end-of-life care by offering emotional support, managing symptoms, and ensuring comfort in accordance with care plans
- Prepare nutritious meals tailored to dietary restrictions and assist with light housekeeping tasks such as cleaning, laundry, and meal preparation to maintain a safe living environment
- Document all patient interactions meticulously in medical records and ensure compliance with home & community care standards
Skills
- Proven experience working with individuals with developmental disabilities or in senior care environments such as assisted living or nursing homes
- Strong knowledge of medical terminology, medication administration procedures, catheter care, and basic life support (BLS) including CPR and first aid certifications
- Familiarity with EMR (Electronic Medical Records) systems and EHR documentation practices for accurate record keeping
- Ability to perform heavy lifting safely when required for patient transfers or mobility assistance
- Excellent patient observation skills to detect changes in condition promptly and accurately report findings
- Compassionate caregiving approach combined with strong communication skills for working effectively with patients, families, and healthcare teams
- Knowledge of home healthcare regulations to ensure compliance during service delivery
- Experience working with geriatric patients or individuals with disabilities in home health or long-term care settings is highly desirable
