PROMOTION OPPORTUNITY: Director of Nursing
Perform admission assessments for new home care clients as assigned.
Coordinate patient admissions in collaboration with the Director of Nursing and Assistant Directors of Nursing.
Obtain all required physician orders within the designated workday to ensure timely initiation and continuation of care.
Ensure all physician orders are complete, accurate, and executable by the DON and/or ADONs.
Work collaboratively with Customer Care Managers (CCMs) to maintain continuity of the authorization process and assist with insurance authorizations when needed.
Communicate directly with insurance providers, Medicare Advantage plans, managed care organizations, and physician offices to facilitate authorizations and obtain required documentation.
Assist with coordination of patient care to ensure timely admissions and continuity of services.
Provide exceptional customer service to patients, families, referral sources, physicians, and interdisciplinary team members.
Current, unrestricted Texas Registered Nurse (RN) license.
Minimum of 2–3 years of nursing experience required.
Previous home health experience is preferred.
Experience working with insurance plans, managed care organizations, and obtaining prior authorizations.
Experience working directly with Medicare Advantage plans, insurance companies, or supporting Customer Care Managers (CCMs) during the authorization process is strongly preferred.
Strong understanding of physician orders, clinical documentation, and regulatory compliance.
Excellent organizational skills with the ability to prioritize multiple tasks and meet deadlines.
Strong communication and interpersonal skills.
Ability to work independently while thriving in a collaborative team environment.
