What You’ll Do
- Provide high‑quality physical therapy services in home health settings.
- Assess functional abilities and help develop individualized plans of care.
- Document accurately and maintain confidentiality.
- Offer education, consultation, and coordination with families and care teams.
- Participate in performance improvement activities and required meetings.
- Uphold PHHI policies, professional standards, and the WV State Practice Act.
What We’re Looking For
- Current West Virginia Physical Therapist license (in good standing).
- Current CPR certification.
- Experience as a Physical Therapist (home health experience a plus).
- Strong critical thinking, judgment, and communication skills.
- Ability to work well with patients, families, and team members.
- Meets all federal program participation requirements.
- Experience with discharge planning and developing comprehensive care plans for diverse patient populations.
- Familiarity with EMR systems for documentation and patient management
- Excellent communication skills to effectively educate patients and coordinate with healthcare teams

Pay: $40.00 - $55.00 per hour
