THE PATIENT TRANSFER CENTER COORDINATOR is to receive and coordinate calls regarding acute patient transfers, consults and/or admission from medical facilities. Documents the transfers and consults from other medical facilities into transfer center program. The Patient Transfer Coordinator utilizes patient logistics software to coordinate patient flow and bed placement. The coordinator speaks directly to the referring and receiving provider/staff to coordinate patient care. Strong communication skills and knowledge of medical conditions and terminology. Coordinates the hospital rotation for Penn Highlands DuBois. The coordinator will also take calls for emergent and non-emergent transportation, schedule and arrange transport via van, ambulance or helicopter (depending on needs) and document all transports as appropriate.
QUALIFICATIONS:

High school diploma or equivalent required

Clinical Medical Assistant, EMT, A-EMT, EMT-P, or LPN required
-
If Clinical Medical Assistant, completion of a Medical Assistant Program with eligibility to sit for NHA, NCCT or AAMA required

Typing and computer skills; ability to prioritize and multi-task duties; ability to effectively communicate with coworkers, physicians, patients and administration required

Must have medical terminology background, strong interpersonal skills, computer literacy and data entry skills
