Siuslaw Outreach Services Job Description
GENERAL RESPONSIBILITIES: Manage all Lane County Basic Needs Vouchers and Self Sufficiency Programs, including Low-income Home Energy Assistance Program, rent assistance programs and other homelessness intervention and prevention support services. Manage the budget, adhere to funding regulations, provide direct services to clients, process and file paperwork, maintain HMIS and OPUS data systems, and all other aspects of these programs. Supervise, train and direct Crisis Assistance Support Staff.
QUALIFICATIONS: Must be able to demonstrate skills in leadership, program management, budgeting, and basic computer skills. Must be able to demonstrate the ability to effectively work with diverse populations including those experiencing homelessness. College degree in social service related field, OR a minimum of 3 years experience in social service field. Possess a valid ODL, and insurable by SOS insurance carrier. Must be able to pass Criminal History check and drug screen.
HOURS/CLASSIFICATION: 1 FTE supervised by Executive Director. Flexible hours apply
COMPENSATION: Starting rate: $21-$22 per hr, 2 week paid vacation, holiday pay, medical & dental insurance. Retirement benefits eligible after one year of employment.
DUTIES AND RESPONSIBILITIES:
· Become knowledgeable regarding each of the funded programs, rules and regulations, reporting requirements, eligibility requirements, budgets and budget reports.
· Set up systems for monitoring funding from each of the programs.
· Set up client file system with all required paperwork in each client file.
· Conduct assessments of potential clients for the programs.
· Complete intakes for clients of the programs.
· Determine services to be provided for each client, develop written case management plans.
· Provide case management, follow up, ongoing meetings with each client.
· Make referrals, as appropriate, for services to victims.
· Coordinate with support staff to conduct case reviews.
· Ensure department staff are documenting and maintaining electronic and hard copy records to comply with funding source requirements and agency policies.
· Network with other local and non-local agencies that provide services useful to clients.
· Train support staff in delivery of Energy and Housing Assistance programs.
· Direct and supervise support staff in delivery of housing and utility assistance programs in order to promote client stability.
· Meet with staff weekly for de-briefing and coordination.
· Meet regularly with Executive Director.
· Maintain accurate records case files and statistical information for required funding sources.
· Observe all confidentiality requirements.
· Attend trainings, as appropriate.
· Coordinate self-sufficiency classes for clients enrolled in specific housing and utility assistance programs.
· Perform related duties, as assigned by Executive Director.
PHYSICAL REQUIREMENTS: Sitting, standing, working at computer, driving, occasional lifting to up to30Lbs.
