Job Location: Charleston, WV and surrounding areas
Position Summary:
Provides initial nursing evaluation and admission (if indicated) on patient referrals to HospiceCare. Coordinates initial services for patients. Performs other job duties as assigned by Care Navigator Supervisor.
Position Qualifications:
- Graduate of an approved nursing program.
- Current unrestricted WV RN license.
- Demonstrate problem solving and organizational skills, effective communication, and interpersonal skills (dealing with patient/family, agency personnel, direct providers, and community) in a professional and positive manner.
- Contingent upon successful completion of drug screen, background check, and other screenings as applicable.
Work Environment:
- Exposure to infectious diseases, bloodborne pathogens, and bodily fluids.
- Frequent interaction with patients, families, and interdisciplinary team members.
- Fast-paced clinical settings with potential for high stress.
- Required use of personal protective equipment (PPE).
Physical Requirements:
- Ability to lift, transfer, and reposition patients up to 50 pounds independently and over 50 pounds with assistance.
- Frequent standing and walking for extended periods (8–12 hour per shift).
- Frequent bending, stooping, kneeling, crouching, and reaching.
- Ability to push and pull equipment (e.g., wheelchairs, stretchers, and medication carts).
- Visual acuity to read medication labels, physician orders, and monitors.
- Auditory ability to assess patient needs and respond to alarms or calls for assistance.
