Job Overview
We are seeking a dedicated and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to provide high-quality primary care services to patients in skilled nursing facilities (SNFs), assisted living facilities (ALFs), and private homes. This is a field-based role focused on delivering comprehensive, patient-centered care in post-acute and home settings. The ideal candidate is self-motivated, comfortable working independently, and passionate about improving healthcare access for vulnerable populations.
Key Responsibilities:
- Conduct in-person medical visits at assigned assisted livings, nursing homes, and home settings
- Perform routine and acute care evaluations, chronic disease management, preventative screenings, and medication management
- Collaborate with facility staff, mobile medical assistants (MAs), and telehealth support personnel to ensure coordinated and efficient care
- Document clinical encounters thoroughly and timely using EHR systems
- Order and interpret labs, diagnostics, and develop comprehensive care plans
- Communicate with family members, caregivers, and interdisciplinary teams as needed
- Adhere to evidence-based guidelines, organizational protocols, and state/federal regulations
- Participate in regular case reviews, team meetings, and quality improvement initiatives
Job Type: Full-time
Pay: $140,000.00 - $180,000.00 per year
