Qualifications and Requirements:
- Current license with the state Board of Nursing
- 1 year of nursing experience is required - home care experience preferred
- Ability to perform tasks involving physical activity, which may include heavy lifting up to 50 lbs, bending, and standing
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
- Ability to successfully complete a background check, professional reference check, and drug/alcohol screening test
Compensation: 72,000-75,000
Pay: $70,000.00 - $75,000.00 per year
Schedule:
- Day shift
- Monday to Friday
Job Summary
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our healthcare team. In this role, you will be responsible for coordinating patient care, ensuring that patients receive the necessary services and support throughout their treatment journey. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a commitment to providing high-quality patient service. This position requires a comprehensive understanding of case management processes and the ability to work collaboratively with healthcare professionals across various specialties.
Responsibilities
- Conduct thorough patient assessments to determine individual needs and develop personalized care plans.
- Coordinate with multidisciplinary teams to ensure effective communication and continuity of care for patients.
- Manage medical records and documentation, ensuring accuracy and compliance with regulatory standards.
- Provide education and support to patients and their families regarding treatment options, resources, and follow-up care.
- Monitor patient progress and adjust care plans as necessary based on ongoing evaluations.
- Advocate for patients' needs within the healthcare system, including referrals to specialists or community resources.
- Collaborate with primary care providers, behavioral health specialists, and other healthcare professionals in managing patient care.
- Participate in quality improvement initiatives aimed at enhancing patient outcomes and service delivery.
