1. Responsible for production and distribution of weekly provider empanelment reports.
2. Determine and designation of open or closed Provider patient panel status based on patient census.
3. Responsible for transition of existing providers panels to new providers/teams.
4. In coordination with Central Scheduling assists with appropriate assignment of patients without a Primary Care Provider (PCP) or patients that need a new PCP.
5. In Athena, reviews charts and manages/maintains unconfigured user work queues (“buckets”).
6. In coordination with provider and CMA Team Leads, draft and dispatch letters for notification of Provider change.
7. Direct responsibility for mailing all written communications relating to patient empanelment.
8. Manages monthly empanelment meeting with Central Scheduling manager, Medical Practice Division Manager, Primary Care Nurse Manager, Clinical Staff Office Director and IT.
9. Works closely with Family Medicine and the Pediatric Program to monitor all pediatric age -related patients that are ready to transfer out of Peds and transition into adult panels.
10. Creation and implementation of a maturity age provider panel transition letter containing information on providers and medical teams.
11. Procurement and provision of quarterly empanelment reports to the Senior Medical Director, Primary Care Medical Director and Medical Division Practice Manager.
12. Weekly review of empanelment spread sheet for PCP/most qualified Provider team assignment. Make necessary changes as appropriate and notify the patient.
13. Regular, accurate, communication with providers regarding panel changes.
14. Reviews and manages PCP Pending report.
