Duties
- Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs and reporting changes promptly
- Administer medications following prescribed protocols and document medical records accurately using Electronic Health Record (EHR) systems
- Support patients with specialized needs such as dementia, Alzheimer's, or developmental disabilities through tailored memory care techniques
- Operate medical equipment like Hoyer lifts, catheters, and other assistive devices safely and effectively
- Provide companionship and emotional support to enhance patient comfort and mental well-being
- Perform light housekeeping tasks including cleaning, laundry, meal preparation, and maintaining a safe environment in compliance with HIPAA regulations
Requirements
- Proven experience in home health care, assisted living, nursing homes or long-term care environments
- Knowledge of medical terminology, medication administration, and vital signs monitoring
- Certifications such as CPR (Cardiopulmonary Resuscitation) and first aid are required
- Ability to work with individuals with disabilities or complex health conditions like dementia or hospice needs
- Familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation
- Strong patient observation skills to identify changes in condition or behavior quickly
- Compassionate attitude with excellent communication skills and a commitment to patient privacy and confidentiality under HIPAA guidelines
