The Executive Director of Marketing and Referral Management is a strategic and operational leader responsible for accelerating market expansion, strengthening referral pipelines, and driving sustainable census growth for Brooks Rehabilitation Home Health. This role oversees sales, marketing, and intake functions, ensuring seamless alignment from referral generation through admission. The position owns key hospital and referral partner relationships and plays a critical role in positioning Brooks as the preferred home health provider across current and new markets.
Key Responsibilities
Growth & Market Expansion

Develop and execute comprehensive growth strategies to expand market share across existing and new service areas.

Identify, evaluate, and lead entry into new markets in partnership with the VP of Home Health and senior leadership.

Analyze market trends, referral patterns, and competitive dynamics to inform strategic decisions.

Set and achieve census, revenue, and referral growth targets aligned with organizational goals.
Referral & Relationship Management

Own and optimize the referral pipeline, ensuring consistent performance from referral receipt through admission.

Build, maintain, and strengthen strategic relationships with hospitals, health systems, physicians, and other referral sources.

Serve as the senior point of contact for key hospital partnerships, including system‑level collaboration and performance reviews.

Sales, Marketing & Intake Leadership

Lead and manage the sales and marketing teams, driving clear accountability, performance, and professional development.

Oversee intake operations to ensure speed, accuracy, compliance, and exceptional referral source and patient experience.

Align sales, marketing, and intake workflows to eliminate friction and maximize conversion rates.

Establish and monitor KPIs related to referrals, admissions, conversion rates, and customer satisfaction.
Operational Excellence & Collaboration

Partner closely with clinical, operations, and finance leaders to ensure growth initiatives are operationally feasible and financially sound.

Qualifications
Required

Bachelor’s degree in healthcare administration, business, marketing, or a related healthcare field (Master’s degree preferred).

8+ years of progressive leadership experience in home health, post‑acute care, or healthcare growth roles.

Demonstrated success driving market expansion, referral growth, and revenue performance.

Experience managing sales, marketing, and/or intake teams in a healthcare setting.

Strong understanding of home health referral dynamics, hospital relationships, and payer environments.
