Youth Haven Services is a comprehensive mental health agency specializing in supporting children, adolescents, and their families. We are seeking a Licensed Professional to join our growing Intensive In-Home (IIH) program as a Team Lead serving clients throughout Forsyth County, NC.
As the IIH Team Lead, you will provide direct clinical services while supporting, coaching, and guiding the IIH team.
The Team Lead is responsible for ensuring high-quality, person-centered services for children and adolescents with behavioral or mental health needs. This role combines direct clinical work with team leadership, supervision, and coordination. Services are provided in clients' homes and throughout the community in Forsyth County, following Youth Haven Services policies and North Carolina state guidelines.
Provide individual and family therapy according to the consumer's Person-Centered Plan.
Complete clinical assessments and reassessments.
Offer clinical guidance and direction to IIH team members.
Conduct weekly team meetings and monitor case progress, interventions, and overall team performance.
Support compliance for intakes, authorizations, reauthorizations, documentation, and productivity.
Track treatment outcomes, ensure timely documentation, and assist with transition and discharge planning.
Collaborate with families, schools, providers, and community partners.
Perform additional duties as assigned.
Master's degree in Social Work, Counseling, Psychology, Marriage & Family Therapy, or a related human services field.
Current North Carolina Licensure or Provisional Licensure (LCSW, LCSWA, LCMHC, LCMHCA, LMFT, LMFTA, etc.).
Minimum of one year of documented clinical experience with the population served.
Valid driver's license, reliable transportation, and current auto insurance.
Ability to travel throughout Forsyth County for home- and community-based services.
Strong communication skills and the ability to maintain professional, ethical behavior.
Ability to engage with and support children, adolescents, and families.
Strong assessment, therapeutic, and crisis intervention skills.
Ability to lead, supervise, and coach team members.
Skilled in documentation, treatment planning, and progress monitoring.
Strong organizational and time-management skills.
Ability to analyze information and develop effective interventions.
