Physical Therapist (PT) – Home Health (Lymphedema)

Position Overview
We are seeking a compassionate and skilled Physical Therapist (PT) with lymphedema treatment experience to join our home health team serving patients throughout the Treasure Coast. This role focuses on delivering high-quality, individualized therapy services in patients’ homes while promoting functional independence, safety, and overall well-being. The ideal candidate is clinically confident, self-motivated, and comfortable working autonomously in a field-based environment.
Key Responsibilities
- Evaluate and develop individualized treatment plans for home health patients.
- Provide skilled physical therapy interventions in accordance with physician orders and care plans.
- Perform lymphedema evaluations and treatment, including manual lymphatic drainage, compression therapy, bandaging, and patient education.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients and caregivers on exercises, mobility techniques, fall prevention, and condition management.
- Complete timely and accurate documentation in EMR system.
- Communicate effectively with physicians, nurses, case managers, and interdisciplinary team members.
- Ensure compliance with Medicare, state, and agency guidelines.
Qualifications
- Current Florida Physical Therapist license in good standing.
- Certified Lymphedema Therapist (CLT) or documented clinical experience treating lymphedema required.
- Minimum 1 year of PT experience (home health preferred).
- Current BLS/CPR certification.
- Valid driver’s license and reliable transportation.
- Strong clinical judgment and documentation skills.
- Ability to work independently and manage a flexible field schedule.
- Home health or geriatric experience.
- Familiarity with OASIS documentation
