A DAY IN THE LIFE OF A HOSPICE QUALITY IMPROVEMENT SPECIALIST
This position is responsible for the review of Clinical staff documentation and to provide guidance in accordance with State and Federal regulations. This position analyzes tracks and trends quality metrics and identifies opportunities for performance improvement and provides input and support for new trainings and education agency wide. Responsibilities include but are not limited to:

Reviews patient records at defined time-points for compliance and accuracy and works with clinicians to ensure they are up-dated, completed and locked in a timely manner and submitted per CMS regulations.

Analyzes data, identifies trends, recommends, develops and assists with the implementation of Performance Improvement Projects (PIP). Prepares written plans and reports for the PIP as appropriate or upon request.

Proactively confers/collaborates with managers/supervisors and other agency personnel on identified areas of opportunity related to but not limited to documentation, processes, regulations, compliance

Organizes and maintains resource materials and various records related to assigned functions. Prepares written plans and reports as needed or upon request.

Actively researches and keeps up-to-date on changes in regulatory requirements, not limited to OASIS, Coding, QAPI, COP's etc. from State and Federal level.
- Assist with coordination of and participates in presenting information for OASIS trainings, clinical staff meetings and other meetings and recourses as requested.
- Acts to assure technical competency of clinical staff by assessing individual learning needs, coordinating learning opportunities in collaboration with Educator and Clinical Managers and evaluating the effect of training and education on practice.

Assist with Hospice and Homecare ADR's in accordance with state and federal regulations

Serves field clinician as directed.
QUALIFICATIONS FOR HOSPICE QUALITY IMPROVEMENT SPECIALIST

Graduation from an accredited school of nursing or therapy (physical, occupational, speech/language) required

Three to five years of home health/hospice experience preferred.
- Prior QI experience preferred.

Ability to obtain valid Vermont State licensure by date of employment required. Valid state driver's license required. Ability to successfully pass motor vehicle record check, child abuse, criminal record check and adult abuse check required.

OASIS certification required.
