Licensed Practical Nurse (LPN) – Outpatient Internal Medicine Clinic
The Licensed Practical Nurse (LPN) provides direct patient care in an outpatient Internal Medicine Clinic, supporting the delivery of comprehensive, patient-centered healthcare to adult and geriatric patients. Working under the direction of licensed providers and as part of the multidisciplinary healthcare team, the LPN performs a variety of clinical, administrative, and patient education duties to ensure quality patient care and efficient clinic operations.
Clinical Patient Care
- Collect patient health information, including chief complaints, symptoms, medical history updates, and reasons for seeking care.
- Obtain and record vital signs, including blood pressure, pulse, respirations, temperature, height, weight, pain assessment, and other required clinical measurements.
- Conduct behavioral health screenings and accurately document findings in the Electronic Health Record (EHR).
- Support care coordination by communicating pertinent patient information to the healthcare team and documenting all interventions in accordance with established clinical guidelines.
- Deliver nursing care based on provider treatment plans, nursing assessments, and each patient's physical, psychological, and emotional needs.
- Assist providers during examinations, treatments, and diagnostic procedures while maintaining proper patient positioning and aseptic technique.
- Prepare and maintain sterile fields and ensure adherence to infection prevention and control standards.
Clinical Procedures
- Administer oxygen therapy, intravenous fluids (as authorized), and prescribed medications through approved routes, including oral, rectal, subcutaneous, and intramuscular administration.
- Perform routine nursing procedures including:
- Electrocardiograms (EKGs)
- Phlebotomy
- Finger-stick glucose testing
- Urinalysis (dipstick)
- Visual acuity screening
- Wound care and dressing changes
- Ear irrigation
- Collection of throat, nasal, eye, and wound cultures
- Application of bandages and other therapeutic dressings
- Clean, inspect, package, and prepare reusable surgical instruments following established sterilization procedures.
- Ensure proper collection, labeling, handling, and transport of laboratory specimens.
- Identify changes in patient condition, recognize medical emergencies, initiate Basic Life Support (BLS)/Cardiopulmonary Resuscitation (CPR) when required, and promptly notify the appropriate healthcare provider.
- Operate, monitor, and maintain clinical equipment including cardiac monitors, defibrillators, and other specialized medical devices.
Immunization Services
- Review patient immunization histories to determine required vaccines in accordance with current clinical guidelines.
- Administer intradermal, subcutaneous, intramuscular, and oral immunizations as authorized by established protocols.
- Maintain accountability for vaccine storage, handling, inventory, and documentation.
- Record all immunizations and patient responses in the Electronic Health Record and applicable tracking systems.
- Recognize and respond appropriately to adverse vaccine reactions, including anaphylaxis, while maintaining current Basic Life Support certification.
Patient Education
- Provide individualized education to patients and caregivers regarding prescribed treatments, medications, disease management, and self-care techniques.
- Instruct patients on medication administration, insulin injections, glucose monitoring, inhaler and spacer use, nebulizer treatments, ostomy care, wound care, and other home management procedures.
- Educate patients regarding expected versus adverse reactions following immunizations and appropriate follow-up care.
- Reinforce preventive health measures and educate patients on recognizing early warning signs of illness.
- Document all patient education provided in the Electronic Health Record.
Administrative and Clinical Support
- Maintain accurate and timely documentation in the Electronic Health Record and other automated healthcare systems.
- Review laboratory and diagnostic test results and notify providers of abnormal findings.
- Communicate provider-directed follow-up instructions to patients and document all patient interactions.
- Review clinical documentation for completeness, coding accuracy, and workload capture requirements.
- Monitor medication, vaccine, and medical supply inventory, including expiration dates, and coordinate replenishment as needed.
- Assist with patient registration, appointment processing, medical record preparation, and other front-office duties when necessary.
- Complete workload reporting and documentation using applicable military and clinical information systems.
- Report adverse medication or vaccine events through appropriate reporting systems, including VAERS when applicable.
- Participate in quality improvement initiatives, mandatory training, competency assessments, and other clinic performance activities.
- Maintain a safe, organized, and professional clinical environment while supporting the mission and objectives of the healthcare team.
