Duties
- Assist clients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility support
- Monitor patients’ health status by checking vital signs, observing symptoms, and reporting changes promptly to healthcare professionals
- Administer medications accurately following care plans and HIPAA regulations to ensure patient safety and confidentiality
- Support clients with meal preparation, feeding, and nutritional needs tailored to individual dietary requirements
- Utilize specialized equipment such as Hoyer lifts and patient transfer methods to ensure safe manual handling and resident transfers
- Provide companionship and emotional support to promote mental well-being and reduce feelings of isolation
- Maintain detailed documentation of care provided, patient progress, and any incidents in compliance with home health standards and regulations
Skills
- Proven experience working with individuals with disabilities, developmental or intellectual disabilities, or neurodevelopmental disorders such as autism or dementia care
- Strong knowledge of patient observation techniques, medication administration, and vital signs monitoring
- Familiarity with assisted living, nursing home, hospice care, and long-term care environments
- Ability to perform manual handling tasks safely using equipment like Hoyer lifts and patient transfer methods
- Excellent communication skills for effective client interaction and coordination with healthcare teams
- Knowledge of HIPAA regulations to ensure patient privacy and confidentiality at all times
- Capable of assisting with personal transportation needs and supporting clients during community outings or appointments
