Job Summary
We are seeking a dedicated and compassionate Case Manager to join our team at Passion Care Agency, specializing in PASA (Personal Assistance Services Agency) programs. In this vital role, you will coordinate and oversee individualized care plans for clients, ensuring seamless access to necessary services and support. Duties
- Conduct comprehensive intake assessments to evaluate client needs and develop personalized care plans aligned with their goals and medical requirement.
- Manage utilization review and utilization management processes to ensure appropriate resource allocation and compliance with healthcare policies
- Facilitate communication between clients, families, hospitals, and insurance providers to streamline case management processes
- Ensure compliance with HIPAA regulations by maintaining confidentiality and protecting patient information at all times
- follow clients health and safety updates and recorded in the computer .
- Strong background in communication with clients and guardians.

- Excellent communication skills for effective collaboration with care givers and clients and guardians.
