Position Overview
Join our team as a SCN-Screener and Navigator! We are looking for a compassionate and detail-oriented Screener/Navigator to be the first friendly point of contact for individuals seeking support services. In this role, you’ll welcome new members, help determine eligibility for programs, gather basic information, and connect members to available resources to ensure they receive the right support from the start.
Initial Screening & Intake
- Conduct eligibility screenings using assessment tools
- Gather demographic, medical, insurance, and social information
- Identify risk factors such as homelessness, food insecurity, unemployment, domestic violence, unmanaged health conditions, transportation issues
Documentation & Compliance
- Maintain accurate records of all collected data
- Ensure documentation meets SCN and Department of Health standards
- Track incomplete screenings and follow up to finish them
Enrollment & Service Coordination
- Help transition eligible individuals into active services
- Coordinate benefits like meals, transportation, personal support programs, health education, specialty services
Ongoing Member Support
- Assist in creating individualized service plans
- Communicate with: care managers, hospitals, behavioral health programs, community organizations
- Track member progress and service usage
Qualifications:
- High School Diploma or equivalent required
- Strong communication and interpersonal skills
- Ability to conduct assessments and gather detailed information
- Strong organization and time management skills
- Ability to work independently
- Bilingual English and Spanish or English and Yiddish
Pay: Starting at $20 per hour BOE
Location: Suffern, NY area
Work Location: In person
Pay: $20.00 - $22.00 per hour
