Job Overview
We are seeking a dedicated and highly skilled Wound Care Nurse / On Call to provide expert wound management and patient care across various healthcare settings. This role requires a proactive professional with extensive clinical experience, capable of delivering comprehensive wound assessments, implementing advanced treatment modalities, and responding promptly to urgent care needs. The ideal candidate will possess a strong foundation in critical care, medical documentation, and interdisciplinary collaboration, ensuring optimal healing outcomes and patient safety. Duties
- Conduct thorough wound assessments utilizing evidence-based practices, including aseptic technique and diagnostic evaluation tools.
- Develop individualized wound care plans incorporating advanced treatments such as Botulinum toxin therapy when appropriate, ensuring adherence to clinical guidelines.
- Manage complex wound cases across diverse settings, including long-term care
- facilities, assisted living communities, hospitals, and home care environments.
- Respond promptly to on-call emergencies, providing immediate clinical intervention for acute wounds, trauma cases, or critical care situations such as trauma medicine or Level I/II trauma center scenarios.
- Administer medications and treatments accurately, including IV infusion, injections, suturing, catheterization, and ostomy care while maintaining strict infection control protocols.
- Collaborate with multidisciplinary teams—including physicians specializing in ophthalmology, cardiology, infectious disease care, and palliative medicine—to coordinate comprehensive patient management.
- Document all patient interactions meticulously within electronic health records (EHR/Epic/Cerner systems), ensuring compliance with HIPAA standards and medical coding requirements such as ICD-10/ICD-9.
- Educate patients and their families on wound healing processes, pain management strategies—including acute pain management—and post-discharge care planning.
Experience
- Proven clinical experience as a registered nurse with a focus on wound care in settings such as hospital medicine, ICU, emergency room, or critical care environments.
- Demonstrated expertise in advanced wound management techniques including suturing, ostomy care, and aseptic procedures.
- Experience working with individuals with disabilities or developmental delays within long-term care or behavioral health contexts is highly desirable.
- Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks to ensure accurate documentation and efficient workflow management.
- Background in managing complex cases involving trauma medicine, infectious disease care, hospice & palliative medicine, or geriatrics enhances the ability to deliver targeted patient interventions.
- Knowledge of medical terminology related to cardiology (cardiac catheterization), pulmonology (ventilator management), nephrology (dialysis), and pediatrics (pediatric trauma) is advantageous.
- Certification in Basic Life Support (BLS) is required; additional certifications such as ACLS or PALS are preferred for handling critical situations effectively.
