A 13-week contract opportunity is available for a Sterile Compounding Pharmacist in Irving, TX, offering two flexible weekday schedule options: Monday through Friday, 8:30 am–5:00 pm or 7:30 am–3:30 pm.
Key Responsibilities:
- Evaluate patients for home infusion therapy eligibility and oversee initiation of home-based care.
- Develop, monitor, and adjust individualized medication therapy plans utilizing clinical assessments, laboratory data, and pharmacokinetic principles.
- Prepare, verify, and dispense sterile medications adhering to USP standards, regulatory requirements, and pharmacy policies.
- Provide education to patients, caregivers, and healthcare providers on medication regimens, infusion techniques, and safe administration.
- Collaborate with healthcare teams to ensure continuity of care and monitor patient outcomes.
- Manage infusion pumps and medication administration devices, ensuring quality assurance and safety.
- Deliver drug information support, maintain accurate documentation, and assist with regulatory compliance and medication safety initiatives.
Minimum Qualifications:
- Doctor of Pharmacy (PharmD) degree or equivalent from an accredited institution.
- Active, unrestricted Pharmacist license in Texas.
- One or more of the following:
- At least one year of sterile IV compounding experience,
- Experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM),
- Completion of an ASHP-accredited Pharmacy Residency,
- Completion of an Advanced Pharmacy Practice Experience (APPE), or
- Equivalent pharmacy experience.
Required Skills:
- Strong clinical expertise in home infusion pharmacy practices.
- Proficient in interpreting laboratory values and applying pharmacokinetic concepts.
- Excellent communication skills with a focus on patient counseling and customer service.
