Responsibilities
- Safely administer prescribed medications to residents following all regulatory guidelines and facility policies
- Monitor residents’ vital signs and overall health status, reporting any changes or concerns to nursing staff promptly
- Assist with activities of daily living such as bathing, dressing, mobility support, and feeding for individuals with developmental disabilities or memory impairments
- Maintain accurate medical records and documentation using electronic health record (EHR) systems like Epic or Cerner
- Support specialized care programs including Alzheimer’s care, dementia care, hospice care, and memory care initiatives
- Follow HIPAA regulations to ensure confidentiality of all patient information and medical records
- Collaborate with interdisciplinary teams to develop personalized care plans that address individual needs and preferences
Experience
- Prior experience working in assisted living facilities, nursing homes, hospitals, or home & community care environments is highly preferred
- Knowledge of medical terminology, physiology, anatomy, and medical documentation practices is essential
- Hands-on experience with EMR/EHR systems such as Epic or Cerner enhances efficiency in patient record management
- Familiarity with tube feeding procedures, catheter care, vital signs monitoring, and medication administration techniques is required
- Background working with individuals with disabilities or complex health conditions like Alzheimer’s disease or dementia is advantageous
- Demonstrated caregiving skills in acute care settings or hospice environments will support success in this role
- Certification as a Medication Aide (CMA) is mandatory; additional certifications in medical office procedures or home & community care are a plus
