Support care coordination and follow-ups
Schedule appointments and manage referrals
Conduct outreach to members, families, and local providers
Enter and maintain accurate documentation in the EHR
Help connect members to medical, behavioral health, and community resources
Strong organization and ability to manage multiple tasks
Clear, accurate documentation
Professional communication with clients and providers
Ability to stay calm with distressed individuals
Follow HIPAA requirements in all interactions
Must live in or within commuting distance of Edenton, NC
High school diploma or GED
Experience in healthcare, behavioral health, social services, or administrative support
Basic computer skills (EHR, email, Microsoft Office or Google Workspace)
Associate’s or Bachelor’s degree (Human Services, Social Work, Psychology)
Experience with Medicaid populations
Bilingual
