Job Summary
We are seeking a dynamic and proactive Wellness Coordinator to join our team.
This role is to ensure that each client’s wellness needs are proactively addressed throughout their stay in the respite program. The wellness coordinator works collaboratively with program staff, case managers, and community medical providers to promote client recovery, compliance with medical orders, continuity of care, and improved health outcomes. This role focuses on helping clients achieve their highest possible level of functioning and developing a strong foundation for managing their health and wellness upon discharge from CCH.
Qualifications:
· Licensed Registered Nurse (RN) or Licensed Practical Nurse (LPN) in good standing with the Oklahoma Board of Nursing.
· Experience in community health, case management, or post-acute care preferred.
· Strong organizational, communication, and problem-solving skills.
· Ability to work independently and collaboratively within a multidisciplinary environment.
· Commitment to the mission and values of Cardinal Community House and the populations it serves.
