Job Summary
SIGN ON BONUS Up To $3,000.00 We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality nursing care to patients in our Skilled Nursing Facility. The LPN will ensure patients receive the best possible care while adhering to established protocols and standards.
Duties
- Administer medications and treatments as prescribed by healthcare providers.
- Perform patient assessments and monitor vital signs to ensure optimal health outcomes.
- Assist in the development and implementation of patient care plans.
- Provide direct patient care, including wound care, catheterization, IV insertion, and tube feeding.
- Document patient information accurately in electronic health records (EHR) systems such as Point Click Care.
- Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.
- Educate patients and families on health management, medication administration, and post-care instructions.
- Maintain compliance with HIPAA regulations and infection control protocols.
- Participate in quality improvement initiatives to enhance patient care services.
Experience
Candidates should possess a strong background in nursing with experience in the Skilled Nursing Home setting preferred:
- SNF or hospital experience preferred, particularly with geriatrics.
- Familiarity with ICD-9/ICD-10 coding and medical terminology.
- Proficiency in administering injections, phlebotomy, and specimen collection/processing.
- Experience working with individuals with disabilities or in hospice care settings.
- Knowledge of acute pain management techniques and case management practices.
- The successful candidate will demonstrate excellent communication skills, a commitment to patient-centered care, and the ability to work effectively within a team-oriented environment.
Job Types: Full-time, Part-time
Pay: Up to $34.00 per hour
Expected hours: 12 – 40 per week
