Mekks Home Health Care LLC is seeking a compassionate and skilled Physical Therapist (PT) to join our growing home health care team. The ideal candidate will provide high-quality, patient-centered care to individuals in their homes, helping them regain mobility, improve physical function, and enhance their overall quality of life.
Key Responsibilities
- Conduct comprehensive physical therapy evaluations in patients’ homes
- Develop individualized treatment plans based on patient needs and goals
- Implement therapeutic exercises, manual therapy, and mobility training
- Monitor patient progress and adjust treatment plans as needed
- Educate patients and family members on exercises, safety, and injury prevention
- Maintain accurate and timely clinical documentation in compliance with state and federal regulations
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated care
- Ensure compliance with all home health care standards and company policies
Qualifications & Requirements
- Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program
- Active Massachusetts Physical Therapist License
- Minimum of 1 year experience (home health experience preferred but not required)
- Strong clinical assessment and patient care skills
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Reliable transportation and valid driver’s license
Preferred Skills
- Experience with geriatric patients and chronic conditions
- Familiarity with electronic medical records (EMR) systems
- Strong organizational and documentation skills
