We are seeking a compassionate and knowledgeable Registered Dietitian to join our interdisciplinary care team at our skilled nursing and long-term care facility. In this role, you'll play a vital part in enhancing the quality of life for our residents through nutrition planning, assessment, and education.
What You'll Do:
Complete nutritional assessments and develop individualized care plans for residents
Monitor and document residents’ nutritional status and adjust plans as needed
Collaborate with nursing, dietary, and medical staff to ensure comprehensive care
Provide in-service education and training to dietary staff on best practices
Ensure compliance with all applicable federal, state, and local regulations
Participate in care plan meetings and quality improvement initiatives
What We’re Looking For:
Current Registered Dietitian (RD/RDN) license in Illinois
Experience in long-term care or skilled nursing settings preferred
Strong communication, organization, and documentation skills
Knowledge of MDS and regulatory requirements (CMS, DOH, etc.)
Passion for improving the health and well-being of elderly residents
