American Traveler is hiring an experienced RN Case Manager for a Discharge Planning position requiring active MA licensure and prior acute care case management experience.

Focus on discharge planning duties

Day shift schedule, Monday through Friday

40 hours per week, with scheduling that may follow a 4x10 or 5x8 format depending on operational need

Shift hours are 07:30 to 18:00

13-week assignment length

Uses Epic for charting

Uses UKG/Kronos for timekeeping

Active MA RN license required; pending or conditional licenses not accepted

Both NURSYS and Board of Nursing verification required for licensure

AHA-issued certifications only

Minimum 2 years of case management experience required

At least 6 months of discharge planning experience in an acute care hospital required

Experience in a large teaching hospital with 500+ beds strongly preferred
-
Flu vaccine required with no religious or medical exemptions accepted

Coordinate discharge planning for patients within the case management department

Local candidates must reside within 60 miles of the facility in Boston, MA
