ABOUT THE ROLE
Aven Care Providers seeks a reliable, organized, and friendly Quality Coordinator to support our day-to-day operations. The Quality Coordinator is responsible for the implementation of quality assurance, performance improvement, and utilization review activities. The Quality Coordinator is expected to spend time in the field with clients periodically to maintain open communication and oversight of assigned caseloads. This position is ideal for someone who enjoys helping people, can work in a fast-paced environment, can manage multiple projects concurrently, and is comfortable with ambiguity.
KEY RESPONSIBILITIES
Quality Assurance Coordination
- Observe and evaluate performance of field staff to ensure quality service, compliance with service plans, and compliance with Agency policies and procedures.
- Investigate and resolve client grievances regarding employee performance and services rendered. If not resolved, provide investigation documentation and recommendations to the Program Director for escalation.
- Coordinate filing of all SIR’s (Special Incident Reports) to Regional Centers, and review and resolve ongoing issues proactively.
- Analyze and record personnel and operational data and provide input to Program Managers for performance evaluations, client progress reviews, and client progress reports.
Performance Improvement Coordination
- Monitors client records concurrently and retrospectively, utilizing agency indicators and thresholds.
- Audits client records for completeness on an ongoing basis.
- Assists the Operations Director with documenting workflows, implementing standardized processes, and other performance improvement initiatives.
- Assists the Operations Director with identifying and logging risks/issues that arise from any quality assurance process, including audit, supervision, incident/SIR review, complaint, grievance, staff report, or individual/team feedback.
- Assists the Operations Director with planning and implementing corrective actions such as retraining, documentation correction, policy revision, supervision, staffing adjustment, or Service Coordinator communication.
- Functions as a resource to agency personnel.
- Maintain confidentiality and follow privacy/HIPAA practices.
Administrative & Data Support
- Assist with general office support tasks as needed, including answering incoming calls professionally and routing inquiries appropriately.
- Perform accurate data entry and maintain logs (issues, incidents, calls, schedule changes, cancellations).
- Coordinate with supervisors and team members to troubleshoot client or field staff issues.
QUALIFICATIONS
Required (Must Have)
- High school diploma or GED
- Ability to manage a high-volume book of work with professionalism, patience, and a positive attitude
- 2 years of experience in healthcare/home care administrative support
- Strong attention to detail and solid project management skills
- Comfortable using Microsoft Office (Outlook, Word, Excel) or similar tools
- Strong organizational skills and ability to multitask in a fast-paced environment
- Reliable transportation
- Ability to maintain confidentiality and follow privacy guidelines
Preferred (Nice to Have)
- Bilingual: English/Spanish
- Experience with AxisCare
- Familiarity with scheduling software, EHR/CRM systems, and quality management systems.
- Familiarity with quality management processes is a plus.
OTHER DETAILS
Job Type: Full-time
Pay: $21-$23 per hour (based on experience)
Schedule: Monday–Friday, 9am-5pm (no after-hours/on-call)
Location: In person (Sherman Oaks, CA)
