POSITION SUMMARY
The Homemaker provides personal assistance and supportive homemaking services to hospice patients in their place of residence, under the Interdisciplinary Group (IDG) Plan of Care and the supervision of the Director of Nursing. The role centers on maintaining a safe, clean, and comfortable home environment — light housekeeping, laundry, errands, and meal preparation — while working cooperatively with the patient and family and reporting observations and concerns to the supervisor.
This is a per diem position. Homemaker services are non-clinical and supportive in nature; hands-on personal care, medication assistance, and clinical treatment are provided by the Hospice Aide and licensed clinical staff. All services are performed in compliance with CMS Conditions of Participation under the supervision of a Registered Nurse.
WHAT YOU’LL DO
- Provide light housekeeping that keeps the patient’s living areas clean, safe, and comfortable, per the Plan of Care
- Perform laundry services, change bed linens, and maintain an orderly patient care area
- Assist with meal preparation and run errands, including grocery shopping, as assigned
- Maintain an environment that promotes respect for the patient’s privacy, dignity, and personal property
- Observe and report changes in the patient’s condition, home environment, or family circumstances to the supervising RN and Director of Nursing to ensure continuity of care
- Practice accepted infection control principles in the home at all times
- Communicate in a positive, supportive, and respectful manner with patients, families, and other team members, and maintain patient confidentiality and HIPAA compliance
- Perform assigned functions according to agency policies and demonstrate effective time management across scheduled visits
- Document all visits in Hospice Tools on the day of each visit
- Attend required in-services and participate in Interdisciplinary Team (IDT) meetings as requested
COMPENSATION
Complete compensation details will be provided in the independent contractor agreement prior to the first visit.
QUALIFICATIONS
Required
- Ability to read and write in English and to follow written and verbal instructions in English effectively
- Demonstrated interest in the welfare of the ill and elderly, and competence to perform tasks assigned by the supervisor
- Ability to work independently in patient homes with minimal supervision
- Valid Texas driver’s license, reliable personal vehicle, and current auto liability insurance for travel throughout Greater Houston
- Ability to pass a criminal history check and all required pre-employment screening
- Current Hepatitis B profile and TB screening in accordance with agency policy
- Smartphone capable of running the Hospice Tools mobile app (iOS or Android)
- Availability to accept per diem assignments on a flexible schedule, including occasional evenings or weekends based on patient need
Preferred
- High school diploma or equivalent
- One (1) year of experience providing services to individuals needing homemaker or companion assistance
- Prior experience in a Medicare-certified hospice or home health agency
- Familiarity with Hospice Tools or a comparable hospice EMR
- Bilingual in English and Spanish
WORK ENVIRONMENT AND PHYSICAL REQUIREMENTS
- Services are performed in patients’ homes under varying conditions, with possible exposure to blood, body fluids, and infectious diseases
- Local travel throughout Greater Houston is required, including some exposure to unpleasant weather
- Prolonged standing and walking; ability to lift and move up to 50 lbs. and to assist with patient positioning as directed
- Requires hand-eye coordination, manual dexterity, and the ability to use durable medical equipment present in the home
- Requires working under occasionally stressful conditions, identifying patient needs, and responding to patient and family concerns

Work Location: On the road
